We are currently open to new patient appointments and welcome the opportunity to participate in your care.
Referrals
To get started, please reach out to your primary care provider or referring provider’s office to request a referral (if required by insurance) and ask them to fax pertinent medical information which may include chart notes, lab results, and/or x-ray reports.
Once we receive these documents, our team will promptly get in touch with you to arrange for an appointment with Dr. Portnoff.
